Healthier Living Program
workshops in your area.
is program is based on Stanford
University’s Chronic Disease Self-
Management Program. e Healthier
Living Program is a six-week series
of workshops where you can learn
to take control of your chronic
health condition. A chronic disease
is an ongoing health condition that
can be controlled but not cured.
Some examples of chronic diseases
are diabetes, asthma, high blood
pressure and heart disease. In these
workshops, you can learn skills to
manage your disease.
Who should attend?
Alliance
members who:
Are 18 years of age and over
Have a chronic disease
Would like to improve their health
and tness and meet new people
Are family and caregivers of an
Alliance member living with a
chronic disease
What topics will be covered?
Topics will include:
Tips to better manage your health
Ways to reduce pain, fatigue
(feeling very tired), stress, anxiety
and sleep loss
Communication skills to work
better with doctors, family and
friends
Goal-setting and problem-solving
Healthy eating and physical
activity
Medication responsibilities
Understanding emotions
Is there a fee?
No. All
workshops are FREE!

You can choose a Spanish or English
workshop in the morning or evening.
Members who attend the six-
week workshop will receive:
A book titled
Living a Healthy Life
with Chronic Conditions
An audio CD called
Relaxation for
Mind and Body

at times you may not know who to
call for what. Here are some tips.
Call the Alliance to:
Learn more about how our
health plan works
Get help with understanding
your health care bene ts and how
to get care
Choose or change your Alliance
Primary Care Provider (PCP)
Let us know if you are not happy
with your provider, the care you
are receiving or something the
Alliance has done
Ask for a new Alliance ID card if
you lose yours

Apply for or renew your Medi-Cal
coverage
Update your address and/or your

Let them know if you have

Notify them if you get or lose
other health care insurance,
besides Medi-Cal
Ask for a new Benefits
