T he Alliance Care Management Program is here to support your health care needs. We want to make sure that our members get the services that they need to stay healthy. Our team is made up of nurses, social workers, health educators and care coordinators. The team’s focus is to work with you, your family, your doctors and other providers to ensure that you receive the care and tools that you need to manage your health. The Care Management Program offers: ■  ■ Case Management Services to assist members who have complex health issues and need help to manage their illness ■  ■ Care Coordination Services for members who have less complex health issues and to connect them with local community resources ■  ■ Health education and chronic disease management programs to help you get healthy and stay healthy To learn more about our Care Management Program, please call the Case Management Line at 1-800-700‑3874, ext. 5512, or the Health Education Line at 1-800-700‑3874, ext. 5580 .
