Community-Based Adult Services C ommunity-Based Adult Services (CBAS) offer services to Alliance members who are older adults with disabilities to help restore or maintain their ability to care for themselves and delay or avoid moving them into a skilled nursing facility. CBAS services include therapy, nursing, social services, socialization and activities, along with a nutritious lunch and transportation. To find out how to get CBAS services, please call a center near you. What is a PCP? T hink of your Primary Care Provider (PCP) as your gateway to the world of health care. He or she will be your first stop when you’re sick and when you need advice about staying well. PCPs can be general practice or family practice doctors, pediatricians, or internists. You can also choose a clinic or medical group to be your PCP. Among other things, your PCP will: ■  ■ Guide all of your health care ■  ■ Be the one you go to for regular care and when you’re sick ■  ■ Help prevent health problems ■  ■ Refer you to other doctors as needed Over time, your PCP will get to know your health history and help you make good choices so you can stay well. We can help you find a doctor One of the best ways to stay well is to work closely with a doctor. If you don’t have one already, we can help. This simple step can lead to a healthier you.
