he Alliance may cover
nutrition formulas or
supplements if you are
losing too much weight because
you have an issue with one of the
following:
Swallowing
Digesting
Absorbing regular foods
To receive nutrition formulas
and supplements, your Primary
Care Provider must send the
Alliance records showing proof of
medical need. If you need a special
formula for a speci c condition or
age, the records must also show
proof of this need.
e Alliance does not cover
nutrition formulas or supplements
when:
You are an inpatient in a hospital
or nursing facility
You are using the formula during
dialysis at a dialysis center
You are able to meet your
nutrition needs with regular foods
(or foods with altered textures)
You prefer the supplement or
formula over regular food
